Arrowhead Co-op PM
Date: June 13 – July 11
Time: 12:20 – 6:00pm
Location: 1800 Vettleson Rd., Hartland
Ages: Children entering 5K – 8th grade
Details:
Participants who are registered through the Arrowhead Area Summer School Cooperative would end the day and travel by bus to Lake Country School and join with the Day Camp that is currently taking place. Participants would not be joining in on Day Camp field trips on Tuesdays and Thursdays, but will participate in the lunch service at Lake Country on Mondays, Wednesdays, and Fridays.

Additional Forms
- Immunization Waiver:
Complete if you are waiving your child’s immunization requirement. - Authorization to Administer Medication: Needed if there is a possibility that our staff will need to administer medication to your child while under our care.
- Alternative Arrival/Release Form: Needed if a parent will not be dropping off/picking up your child from our program (walks to school, leaves to attend another program, etc.)
- Y Care Bank Draft Form: If you need to update your bank draft information.
